This is your kind information that I kaushalendra singh s/o shri devendra singh went to Paras Hospital for Head pain medicine and Doctor Was Dr.Sonali Jain and After detail discussion she suggest & admitted in Paras hospital under Neuro consultant Dr.Rajesh Garg and Dr.Tina for Head Pain on dated 20/06/2013 to 22/06/2013 through my Mediclaim policy issued by National Insurance and TPA with PARK MEDICLAIM TPA PVT LIMITED & Policy number is 361100/48/13/8500001332. Park mediclain has declined the insurance claim improper submission of claim paper by Paras Hospital. The treatment money has been made in cash due to insurance claim declined by Park mediclaim tpa pvt ltd. As I am having one another the medicalim card of Apolo Munish and request to Paras hospital doctor team to get the claim through this card but the request has been declined by Paras hospital doctor team.
