Sir/Madam,
With due regards, I am to inform you that my daughter made a bhartiaxalife monthly income plan (Policy no: 5009366344 ) whose premium due date was 28/12/13. This is as mode of annual policy.
The premium amount is :premium annual 14775.09
Nominee was myself: Sonali Senapati
Unfortunately my daughter died at Genesis Hospital Kolkata on 09/11/2013.
I tried several times to catch the officials of BhartiAXA in Kolkata. But, in some places at Kolkata the office does not exist and I have not got any co-operation from Mr. Pushan Das (09836151955) and Mr. Rudra and others.
Let me tell you that my daughter,Late Piyali Senapati, was a BhartiAXA Life Advisor also till death.Under this circumstances I think you can understand my mental ability.
I do make a complaint against BhartiAXA Life Insurance so that this BhartiAXA offers me the claim amount as I was the nominee,Sonali Senapati.
Looking forward for your immediate response. Kindly tell me the steps to be followed.
