Dear Sir, This is to inform you that that i have taken a health insurance in march 2014 from SBI helath isurance but it have been passed around 9 month has been passed but still i did not received my cashless card. then sent only the document regarding the same. I complained many time at the branch and every time they assured me that you will receive it within one week but till the date i did not get this.
So Its my humble request to you please take the necessary action regarding the same.
and for any information regarding the same please inform me.
